In another study, Hrushesky and his colleagues reviewed the records of 44 premenopausal women with early stage breast cancer five to 12 years after breast-removal surgery. Their findings suggest that the timing of surgery for breast cancer can determine its long-term success. Women whose surgery took place mid-cycle, that is, near ovulation, had fewer and later recurrences, and they lived longer than those who were operated on at the time of menstruation. The hormone estrogen, which is highest mid-cycle and lowest during menstruation, may exert a protective effect, Hrushesky speculates. "Surgical timing," he asserts, "is second only to tumor size and nodal status in breast cancer survival."

Better understanding of the rhythmic action of various drugs may lead to

the development of both more effective drugs and more effective ways to

deliver drugs. The chief pineal hormone, melatonin, for example, is synthesized and secreted only in the dark. "Because it appears to be a good sleep-inducing agent," says Russel Reiter, PhD, professor of neuroendo- crinology, researchers are exploring its usefulness to combat jet lag and assist sleep in rotating shift workers when they work at night and must sleep during the day.
